Long Term Disability

General Information
Disability benefits protect you and your family from financial hardship due to an illness or injury that prevents you from working. The Long-Term Disability (LTD Plan pays a benefit of 60% of your eligible pre-disability monthly earnings for medically certified disabilities lasting more than six months. The maximum LTD benefit is $15,000 per month.

Eligibility
Only full-time employees are eligible for LTD benefits. New employees must elect coverage within 31 days of their hire date. If coverage is not elected during this initial eligibility period, you will be required to provide evidence of insurability and receive approval from Liberty Mutual should you wish to enroll during a future open enrollment period. The cost of coverage is based on your pay and age, and is paid through payroll deductions.

How to Apply for LTD Benefits
If you are receiving Short-Term Disability (STD) benefits during the first six months of your disability, your case will automatically transition to Lincoln Financial’s LTD department. Lincoln Financial will contact you directly to complete your application for benefits.
